There are many legal forms that must be completed for burial and cremation services. For your convenience, we have included access to these forms so that you can have the option of completing them at home with the assistance of family members.
The following are instructions on how to complete these forms online. Click on the form link below and an online form will open that will allow you to type in the answers to the form. Areas on the form that you cannot enter information will be filled in by the funeral director.
Note: To make sure that you do not lose any information, please select File / Save As to save the completed or partially completed document to your computer. You can then later open this document to complete or make changes.
When you have completed the documents, follow the instructions below.
- Print forms
- Sign/initial in the spaces designated by the yellow highlighted text on the rest of the pages in front of a Notary Public.
- Have the Notary Public fill out and sign the Cremation Authorization Supplement Form Facsimile Transmission Approval Sheet.
- Fax the documents to us at (979) 823-8050 or e-mail them to firstname.lastname@example.org.
